When you complete our quote form and check the health-information box, we collect:
These answers are linked to the identifying and household information you enter on the same form: name, phone number, email, ZIP code, date of birth, age, household size, and household income.
Directly and only from you — the answers you type or select in our quote form. We do not buy, infer, or derive health information about you from any other source.
We use your health answers for one purpose: matching you with, and selling your information to, one licensed insurance agent who can quote and enroll you in health coverage. The agent pays us a fee for your information. We do not sell your health information to data brokers, and we do not use it for unrelated marketing.
The agent who buys your information may share it with an insurance carrier in order to process a quote or an application on your behalf.
Selling your health information requires a separate, unchecked consent box on our form. If you leave the box unchecked, any health question you may have answered stays on your device — we do not receive, store, or sell it, and the remaining health questions are skipped. You can still request a quote — your request is matched using your non-health answers (location, age, household, income).
Email privacy@leadmachinellc.com from the email address you used on the form and tell us you revoke your health-data authorization, want your health information deleted, or both. We will honor a revocation going forward and process a deletion request for the information we hold. If your information has already been sold to an agent, we will tell you which licensed agent received it so you can direct a deletion request to them as well.
You can also stop communications at any time: reply STOP to any text, or say "stop calling" on any call.
